Due to the State updating forms from time to time the Administration Office requests that any inquires are directed to the office. This will ensure that a person or business will receive the correct and most up-to-date forms needed for the license they are wanting to apply for. All license applications require background checks. Anyone with ownership in the business is required to have a background check completed. The fees charged are the costs for running the background check through a service utilized by the City and the fees charged are non-refundable.
